Complete the Registration Form Below to Join Our Summer Camp

I, the undersigned, as parent or legal guardian of the above-named camper, hereby give permission for my child to participate in the 2025 Summer Art Camp at October Sun Art Studio. I understand that participation in this program may involve physical activity, use of art supplies and tools, and other normal camp activities.

I release and hold harmless October Sun Art Studio, Jeanne Henderson, staff, volunteers, and all affiliated individuals or organizations from any and all liability for injuries, illness, accidents, or loss of property that may occur as a result of participation, whether on-site or off-site.

I authorize October Sun Art Studio staff to obtain emergency medical treatment for my child should it be necessary. I understand every effort will be made to contact me first.
